*Job Summary:*
We are seeking a dedicated and qualified Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to join our healthcare team in Penn Yan, NY.
The ideal candidate will provide comprehensive nursing care to patients while collaborating with healthcare professionals to deliver quality healthcare services.
As an LPN/LVN, you will work closely with physicians, registered nurses, and other healthcare professionals to ensure the highest level of patient care is consistently delivered.
*Responsibilities:*
- Assess patients' health condition and develop individualized care plans based on their needs and goals.
- Administer medication, injections, and other treatments as prescribed by healthcare professionals.
- Monitor patients' vital signs and promptly report any changes or concerns to the respective healthcare provider.
- Educate patients and their families on effective healthcare practices, treatments, and medication management.
- Assist in the coordination of patient admissions, transfers, and discharges.
- Maintain accurate and up-to-date medical records, ensuring compliance with all regulatory standards.
- Collaborate with interdisciplinary healthcare teams to provide efficient and effective patient care.
- Adhere to infection control policies and procedures to prevent the spread of infectious diseases.
- Regularly communicate and report patient progress, concerns, or changes to the appropriate healthcare professionals.
- Provide emotional support to patients and their families, demonstrating empathy and compassion.
*Requirements:*
- Valid LPN/LVN license issued by the State Board of Nursing in New York.
- Proven work experience as an LPN/LVN, preferably in a hospital, clinic, or skilled nursing environment.
- Excellent knowledge of nursing principles and practices, including infection control protocols.
- Strong assessment, critical thinking, and decision-making abilities.
- Effective communication and interpersonal skills to interact with patients, families, and healthcare professionals.
- Ability to work efficiently in a fast-paced environment while maintaining attention to detail.
- Proficient computer skills to document patient information and access electronic medical records.
- Physical stamina and ability to effectively manage time and prioritize tasks.
- Willingness to work flexible hours, including weekends, evenings, and holidays.
