*POSITION DESCRIPTION* The Charge Nurse provides and is responsible for care to patients in the alcoholism and/or substance use disorder inpatient environment.
The Charge Nurse is responsible for supervision over the nursing staff and overall client medical care.
All client interactions must be conducted in accordance with inpatient regulations as required by the NYS Office of Alcohol and Substance Abuse Services (OASAS).
*Essential Job Functions* * Supervises nursing staff at Inpatient by assigning tasks based on individual competencies, evaluation performance and making recommendations concerning performance to maintain a high level of quality care.
Participates in the Progressive Disciplinary Process.
* Responsible for scheduling and nursing coverage * Assesses, evaluate, and respond to changes in client conditions, documenting and reporting such changes to appropriate person to assure prompt response to client needs.
* Assures client care meets best practice standards and in compliance with protocols for the unit.
* Provides medical/behavioral health monitoring of clients and utilizes protocols as ordered by physician.
* Assess the acuity of unit and bring concerns to the attention of the System Director of Nursing or the Medical Provider on Call.
* Conducts and/or participates in admissions, discharges, and transfers and orientation to the unit as necessary and in accordance with program protocols.
* Administer routine medications according to the nursing standards of care.
* Communicates effectively with physicians and other medical providers on client care for SUD, psychiatric and other medical needs.
* Transcribes orders, maintain controlled medication counts and document to maintain accurate client records.
* Monitor and treat clients who have a concomitant substance use/medical/psychiatric illness.
* Document pertinent information in accordance with established standard protocols and procedures.
* Works in conjunction with other nurses from all shifts to communicate client’s needs to improve client care.
* Meets with other team members and participates in developing nursing care plans for each individual client to best meet the needs of the client.
* Actively participates in the Interdisciplinary Team, conducting rounds, and modifying Treatment Plans * Assures compliance with all governing regulations, policies, and procedures*.
* * Responds to all emergencies and provides direct client care in a professional, skilled manner.
* In conjunction with the System Director of Nursing, conducts monthly unit meetings, and distributes all necessary information to staff.
* Assess the individual educational needs of the staff and takes opportunities as available to educate staff to promote improved client care.
* Participates in the evaluation of staff performance and provide assistance and education, as needed, to assist them in providing high quality, individualized nursing care.
* Demonstrates responsibility regarding punctuality and attendance to assure staff is available to provide client care.
* Attend training to update knowledge base and enhance both professional and personal growth.
* Provides instruction to other nursing staff by example and explanation to ensure quality care to clients and to maintain professional standards of nursing.
* Monitor and provide emergency medical care and daily medical treatments to medically compromised clients.
* Oversees the monitoring of inventory, ordering of medications, and equipment needed for daily operation of the unit.
* Functions as part of the practice environment health care team by developing medical intervention programs designed to meet the needs of the client.
* Participates in intake meetings.
* Acts as the Inpatient health care coordinator with responsibilities to ensure the provision of education, risk reduction, counseling and referral services to all residents regarding HIV, AIDS, tuberculosis, hepatitis, sexually transmitted diseases and other communicable diseases.
* Oversees medical staff to ensure medication management, including accurate and timely documentation and program participant education is conducted according to policy.
* Assists and trains staff in counting controlled medications.
* Participate in program on call.
*Secondary Job Functions* * Consistently maintains compliance with regulations governing HIV: NYS Public Health Law, NYS OASAS Regulations, 42CFR, and other patient confidentiality requirements * Completes required medical record documentation in a timely and comprehensive manner and in compliance with NYS OASAS regulations.
Effectively utilizes Agency’s medical software program(s) * Participates as a contributing member of an interdisciplinary team in coordination and delivery of patient care * Participates in professional supervision and education that supports continuous improvement of client care, including weekly case management staff meetings * Reviews disaster plan once a year, or as needed, to ensure medication/medical compliance in the event of an emergency.
* Maintains QHP status as required by job title * Perform other related duties as assigned *Knowledge, Skills and Abilities* * Familiarity in formatting and revising individual treatment plans in collaboration with multidisciplinary treatment team.
* Knowledge of harm reduction and medication –assisted therapies.
* Knowledge of principles & practices of supervision, performance evaluation, training needs and corrective action.
* Ability to relate well with others; demonstrated professional interpersonal skills.
* Knowledge of HIPAA Regulations as well as Federal Regulation 42 CFR Part 2 and Part 8.
* Knowledge of the types and application of emergency medical procedures such as cardiopulmonary resuscitation (CPR) and tourniquets.
* Knowledge of human anatomy and physiology as applied to nursing.
* Knowledge of standard nursing policies, procedures, methods, and techniques.
* Knowledge of the types and characteristics of drugs used in diagnosing, treating, and controlling physical, mental and/or emotional disorders.
* Ability to read and interpret documents such as physicians’ orders and treatment plans.
* Ability to participate in a multi-disciplinary health care team.
* Knowledge of the care and treatment and natural course of substance use disorders.
* Knowledge of substance use and treatment modalities.
* Knowledge of chemically addicted dual diagnosed (substance use/mental health) or psychiatrically ill clients with co-morbidity*.

* The employee may lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds The Charge Nurse must be able to prioritize situations and manage time and may find that they are needed at irregular hours causing fatigue and stress.
The RN may experience stress and stress related symptoms due to interacting with clients in crisis.
S/he may be exposed to illness and unsanitary conditions so must take care to protect their own health.
They may also have to manage physically threatening clients and be prepared to be responsible for their own safety, and the safety of others.
*Work Environment* The employee is expected to adhere to strict policies/regulations relative to being in a tobacco and alcohol free workplace.
Work is performed in an SUD Residential Unit.
*Qualifications* Minimum of an Associate’s degree in Nursing, with prior clinical experience in a substance use disorder environment or related field and 2 years of supervisory experience, a New York State Education Department License as a Registered Nurse, a valid driver’s license.
