Highland Park Rehabilitation and Nursing is currently accepting applications for an experienced Program Coordinator to join our team!
Salary: $30- $35 per hour
As the Program Coordinator you will be responsible to:
Assess each applicant for admission to the program.
Coordinate all services of adult day health care with nursing home.
Schedule adequate staff to provide safe care for registrants.
Hire, discipline, evaluate, all staff.
Arrange inpatient and/or other services for registrants no longer able to participate in the program.
Develop and revise all policies and procedures for program, in conjunction with nursing home.
Assist staff RN with completion of comprehensive care plans.
Promote teamwork, individualized care, respect and professionalism.
Demonstrates enthusiasm and general interest in work.
Job Requirements:
RN License required
Excellent communication and written skills
Minimum of one year experience as a Nurse with community agency or in a long term care setting
Minimum one year management/supervisory experience
Received the COVID-19 vaccination is a must
