“Celebrating Abilities” House Supervisor TITLE: House Supervisor.
STATUS: Hourly, Non-exempt.
PURPOSE: To serve as an immediate supervisor, in cooperation with the Assistant Residential Director, for the Direct Support Professionals in a residence or residences at Lochland, including assisting with documentation, oversight of records, and direct care, as needed.
ACCOUNTABILITY: The House Supervisor is employed by Lochland, Inc.
and is accountable to the Deputy Director through the Team Leader, Residential Director, and the Assistant Residential Director.
REPORTS TO: The Assistant Residential Director.
JOB RESPONSIBILITIES, TASKS, AND PERFORMANCE RESULTS EXPECTED: Function: Administration Percentage of time spent: 40% Tasks: The House Supervisor will ensure that all documentation required from the Direct Support Professionals (including residential habilitation goals, continuous notes, daily assignment sheets, census sheets, bed checks, community activity forms, and medical documentation) is complete and up-to-date, in both Medisked and in any written form, as required.
The House Supervisor will prepare the Daily Assignment Sheet, with the Assistant Residential Director, prior to any shift for days assigned.
The House Supervisor will coordinate with the Residential Director to maintain an accurate daily schedule for the Direct Support Professionals.
The House Supervisor will ensure that all information is entered in the Communication Log, as necessary.
The House Supervisor will remind Direct Support Professionals to complete all Read and Signs within the specified time.
The House Supervisor will complete the Rounds Checklist to ensure all documentation is being completed daily.
The House Supervisor will call each residence two times per shift to receive status reports, completing all documentation of these calls.
The House Supervisor will attend the weekly Managers’ Meetings, individual meeting with the Residential Director, and all-staff meetings.
The House Supervisor will attend all scheduled training events.
With the residents and Direct Support Professionals, the House Supervisor will develop a monthly schedule of activities for the residents.
The House Supervisor will complete any other assigned tasks as requested by a supervisor.
Performance meets expectations when: The House Supervisor reviews all required documentation before the end of his/her shift daily.
The House Supervisor maintains Daily Assignment for review by the Assistant Residential Director at the completion of each month.
The House Supervisor maintains an accurate daily schedule for the Direct Support Professionals that can be accessed by those who need the information.
All information is entered in the Communication Log in a timely manner, as required.
Read and Signs are completed by the twenty-first (21st) day following distribution.
The Rounds Checklist is completed thoroughly be the end of each shift.
The information from the phone calls to the residences twice each shift is recorded at the bottom of the Rounds Checklist following each phone call.
The House Supervisor attends all weekly Managers’ Meetings, meetings with the Residential Director, and all-staff meetings, unless expressly excused by his/her supervisor.
The House Supervisor attends all scheduled training events and maintains current training expectations.
With the residents and Direct Support Professionals, the House Supervisor develops a monthly schedule of activities for the residents no later than the twentieth (20th) of the month prior to the scheduled activities.
All requested tasks are thoroughly completed by the deadlines given by his/her supervisor.
Function: Supervision and Training Percentage of time spent: 30% Tasks: The House Supervisor will supervise the Direct Support Professionals working in the residence or residences daily, in cooperation with the Assistant Residential Director.
The House Supervisor, in cooperation with the Residential Director, will fill any vacancies in the Direct Support Professionals’ schedule for the residence or residences, working to minimize the allocation of overtime hours and assignments.
The House Supervisor, in cooperation with the Residential Director, will provide progressive guidance for Direct Support Professionals, as required.
The House Supervisor will ensure that new staff are thoroughly trained on the resident-specific training requirements for everyone in the residence or residences.
The House Supervisor will ensure that new staff are thoroughly trained on the policies and procedures specific to the location or locations in which staff is placed.
The House Supervisor will work with the Residential Director to ensure that all Direct Support Professionals maintain up-to-date training as required by Lochland, Inc.
Performance meets expectations when: The House Supervisor provides adequate supervision of Direct Support Professionals thereby minimizing incidents, conflicts, and resident complaints.
The House Supervisor sets and meets goals to minimize the allocation of overtime hours and assignments.
The House Supervisor completes any documentation from the all-staff meeting as requested by his/her supervisor.
The House Supervisor completes any necessary progressive guidance for Direct Support Professionals within five (5) days of the discovery of an issue.
The House Supervisor completes all resident-specific training paperwork with new staff during the three-day training period.
The House Supervisor completes all training paperwork on the policies and procedures specific to the location or locations with new staff during the three-day training period The House Supervisor informs Direct Support Professionals about any training requirements for the upcoming month by the end of each month prior to the offered trainings.
Function: Direct Care Percentage of time spent: 30% Tasks: The House Supervisor will assist residents with daily living skills and special challenges, including providing personal care, as required.
The House Supervisor will supervise and participate in daily activities, which may include transporting individuals to community activities.
The House Supervisor will work with residents to complete goals as identified in the resident’s Residential Habilitation Plan.
The House Supervisor will ensure that each resident is supervised and provided care as outlined in the resident’s Individual Plan of Protection, Behavior Support Plan, Medication Monitoring Plan, Nursing Care Plan, and any other relevant documents.
The House Supervisor will provide services to the residents consistent with the responsibilities identified in the Core Competencies Evaluation developed by OPWDD.
The House Supervisor will maintain his/her AMAP certification.
The House Supervisor will perform all other duties as assigned.
Performance meets expectations when: The House Supervisor receives a satisfactory evaluation on the Core Competencies Evaluation developed by OPWDD.
The House Supervisor adequately maintain his/her AMAP certification.
EVALUATION: In addition to ongoing feedback from supervisors, there is a biannual evaluation of performance, on the Core Competencies Evaluation, completed by the Residential Director.
EMPLOYMENT TERMS: This is a full-time position for forty hours per week.
The employee must maintain confidentiality at all times.
