Purpose: To coordinate appropriate support services and resources throughout UPMC to provide effective quality care considering age specific needs and achieve optimal satisfaction, clinical and financial outcomes along the defined continuum of care.
Responsibilities: Works with the RN Care Manager to coordinate appropriate services and resources to ensure admission appropriateness and timely resource utilization according to the Plan of Care.
Possesses the ability to obtain and interpret information considering age specific needs.
Screens all patients to identify patients with physical, psychological and social needs.
Ensures coordination, referral, documentation and follow up contact (as needed) of the discharge plan and communicates to the patient, family and multidisciplinary treatment team regarding the next step in the continuum of care.
Evaluates trends in patient/physician/staff satisfaction, makes appropriate recommendations.
Provides patient counseling, trauma case evaluation, screening for abuse and crisis intervention as appropriate, including facilitation and coordination of patient/family conferences when needed.
Demonstrates commitment to Service Excellence by understanding the needs of patients, their families, physicians and co-workers and doing what is required the first time, every time by being responsible, sensitive, accurate, timely, coordinated and thorough.
Maintains awareness of PRO and payer activities, regulatory standards, available community resources and high level of knowledge of professional practice issues.
Conducts focused assessment of the behavioral health population.
Coordinates appropriate referrals, 302 and 201 commitments and facilitates safe discharge of in-house patients to designated psychiatric settings.
Identifies, provides and / or refers patients who need financial counseling and provides assistance as appropriate Resolves potential patient specific avoidable delays and influences operational system changes.
Works with the RN Care Manager to monitor and intervene with observation services and continuing medical necessity issues.
A Bachelor's degree in social work, psychology, sociology, or other field related to social work is required.
Three years current experience in the acute care setting preffered Current demonstrated competence in appropriate/relevant clinical areas.
Further, demonstrated competence in, but not limited to, the following skills: communications, coordination, problem solving/critical thinking, hospitality/public relations, team facilitation/collaboration, conflict resolution, flexibility, creativity, persistency and tenacity.
